New Patient Registration

In order to register with the practice you will have to provide the following information:

  • A form of identification – passport.
  • Proof of address, e.g. utility bill, bank statement

The receptionist will ask you to complete a registration form. Once completed, your details will be added onto the computer and you will become a member of our practice. You will then be asked to make an appointment with our practice nurse team for a general health check up. Patients on potentially abusable medications may need to see their registered GP prior to a decision being made on whether these medications will be continued or not.

PLEASE NOTE this practice does not prescribe Methadone (or suboxone), but would refer any patients on this medication to the specialist substance misuse services. Any patients affected by this who wish to register at this practice must ensure they have alternative arrangements in place until they are seen by the specialist services.
